STANDARD OF CARE

CARE PLANS

A care plan is used to identify the resident’s needs, the aims of the care to be given, action taken to help meet those needs and the outcome of care.  Care plans are a working document and may take up to 72 hours after arrival to reflect the persons needs.

The care plans are reviewed in accordance with the terms set out in the care contract.  Responsibility for arranging the review will be with the admitting social worker.  Privately funded resident’s care plans will be reviewed as an on-going discipline and is the responsibility of the senior carer.

Should you wish to discuss the care plan  of a individual prior consent would be required by that individual.  Consent of the resident may not be required in certain cases.  Please ask to speak with the manager or senior carer who are always available.

SPECIAL NEEDS

If a resident has any special needs these should be identified on admission and entered into the care plan.  Any previous reaction to any food (nut allergies etc) should be registered on admission to Collyhurst. We endeavour to address any special requests through diet, facilities or equipment.  However, if new needs develop after admission, it will be reviewed and assessed at that time to see if they can be met.

DIGNITY

A major element in achieving this objective is personal appearance.  The home does not provide personal clothing or toiletries and we request supporting relatives to maintain the replacement of clothing as necessary.  Feeling good is also enhanced by deodorants, perfume, after shave and make up.  If you provide these, we will endeavour to ensure that they are used.
